    1. Initial Application - If this is the first time you are applying, check this box.
    2. Upgrade - If you are upgrading your classification, check this box.
    3. Interchange - if you are moving from the Third Area Qualification on to the Southern California Qualification List, check this box.
  3. Project:
    1. Complete each line of the details of the project.
  4. Camera:
    1. 'S' = single camera projects.
    2. 'M' = multicamera sitcoms. Projects involving 'A' and 'B' cameras are considered Single camera.
  5. Media:
    1. F = Film
    2. T = Tape
    3. HD = Hi Def
  6. Type of Project:
    1. F = Feature
    2. E = Episodic
    3. MTV = Movie for Television
    4. S = Special
    5. V = Variety
    6. N = News
    7. G = Game Show
    8. O = Other
  7. Job Title:
      Enter the title used on your project deal memo.
  8. Dates of Employment:
    1. "From:" enter your start date
    2. "To:" Your last day of shooting or last day of wrap
  9. Number of Days Worked:
    1. Prep/Reh = Exact number of paid days you worked prior to all photography.
    2. Shoot = Number of all paid days you worked on set during all photograpy. Camera test days count as shoot days. Still photography days do not count.
    3. Studio - Any days worked on a sound stage.
    4. Lcl Loc - Number of days worked off a studio lot or not on a sound stage.
    5. Dis Loc - Locations when the company was engaged in over night stays.
    6. Wrap - Exact number of paid days worked after completion of photography.
  10. Documentation:
      Check each box for any and all documentation provided with application.
  11. Page Total:
      Sub totals on each page of columns A, B, & C.
  12. Grand Total:
      Total of all days submitted in each of the Prep, Shoot and Wrap columns.
